CLARK UNIVERSITY SPORTS MEDICINE

The sports medicine staff, in conjunction with its facilities, will provide professional allied health care coverage to all sponsored varsity intercollegiate athletic teams, throughout the entire school year. The sports medicine staff at Clark University is comprised of two full-time athletic trainers, and one split appointment athletic trainer/strength and conditioning coach. It is the goal of the staff to provide health care coverage with equal access to prevention, evaluation, treatment, and rehabilitation services of athletic injuries for all student-athletes while competing for Clark University.

MEDICAL CLEARANCE

For New Student-Athletes

Please complete documents online and scan and upload a copy of your physical. You may register or access your ATS profile at clarku2.atsusers.com.
All paperwork is due by the Friday of the first full week of August.

Instructions for registering with ATS and completing forms. (New Students)

Pre-Participation Evaluation (must be within 6 months before your first day of practice and ONLY this form will be accepted!)

Concussion Protocol

Your physical may not be older than 6 months before your first day of practice. General start dates are the end of August for fall sports, the beginning of September for all others.

Necessary Paperwork to be Completed on ATS:

  2. Sickle Cell Trait Waiver
  3. Concussion Policy Form
  4. Assumption of Risk
  5. Insurance Coverage Agreement
  6. Release of Medical and Treatment Information
  7. Insurance Information & Picture of Card
  8. Emergency Contact Information
  9. ADHD Form (only if taking applicable medication)
  10. Doctor's clearance notes for any unresolved injuries or summer surgeries.

For Returning Student-Athletes

Instructions for Completing ATS Documents (Returners)

Necessary Paperwork to be Completed on ATS:

  1. Annual Medical History

  6. Release of Medical and Treatment Information
  7. Check insurance and contact information for accuracy
  8. Doctor's clearance notes from any injuries that were not resolved during the previous school year or summer surgeries.

Inclement Weather Policy and Procedure

 It is vital for the safety of our student-athletes, coaches, staff, and spectators that the Inclement Weather Policy and procedure be implemented. Mandatory compliance with this policy and procedure must be met to ensure the safety of all involved. Hours of Operation During School Year

Hours of operation will vary depending on the practice/game schedule. Hours are posted outside of the Kneller Athletic Center Athletic Training Room.
The athletic trainer covering the practice will be available at the closest athletic training room to the practice 1 hour before it begins.
When there are no practices occurring, the Kneller Athletic Training Room will be staffed for treatment/evaluation appointments.

Dolan Athletic Training Room: As Needed
Weekends: Event/Practice Coverage only.

Directions to Kneller Athletic Center:

From the East and West: Take Massachusetts Turnpike I-90 To Exit 10A Route 146 North. Take 146 North until it ends underneath the 290 overpass. Take a left onto Cambridge Street and continue for a few miles to the fourth set of lights where you turn right onto Main Street at Webster Square. At the first light, turn left onto Maywood Street. At the first stop sign take a right onto Florence Street. At the next stop sign take a right onto Downing Street. Downing Street leads you into The Kneller Athletic Center parking garage.

From The North: I-495 South to I-290 West, Exit 25B. From I-290 West take Exit 11 (College Square, Southbridge Street). Bear left coming off the ramp onto Southbridge Street. Continue straight ahead on Southbridge Street to the second stoplight and take a sharp left onto Cambridge Street. At the third light – a few miles – turn right onto Main Street at Webster Square. At the first light, turn left onto Maywood Street. At the first stop sign take a right onto Florence Street. At the next stop sign take a right onto Downing Street. Downing Street leads you into The Kneller Athletic Center parking garage.

 


Kneller Athletic Center Evacuation Plan:

  1. In case of fire or emergency evacuation, pull the fire alarm. University Police receives an alarm signal. Sprinkler heads will not turn on unless intense heat activates them.
  2. All Athletic staff members in the building meet at the bottom of the stairs next to the main gym floor.
  3. Highest level administrator will quickly designate who will search for key areas of building to evacuate people. Search areas include:
    1. Gymnasium
    2. Athletic training room
    3. Multipurpose room
    4. Conference rooms
    5. SID office
    6. Fitness center
    7. Crew room
    8. Locker rooms
    9. Racquetball/squash courts
    10. Pool
    11. Varsity weight room
  1. Staff members will carry out assignments evacuating people in building along the way to the nearest exits.
  2. Meeting place of all building occupants is the walkway to the Fuller Quad outside of the Kneller between Wright Hall and Dodd Hall.
  3. Administrators will take a headcount of staff members given assignments.
  4. Staff will consult each evacuee about whereabouts of others who may have been in the building.
