Athletics Clearance Process For First-Years & Transfer Varsity Athletes

All incoming first year and transfer Student Athletes must complete two sets of forms - Medical forms and NCAA forms. Below are detailed directions showing how to complete your paperwork.

Medical Paperwork

Start by registering for Athletic Trainer System (ATS) software at clarku2.atsusers.com. Click here for instructions on how to register and use ATS.  

Medical Clearance Checklist - Complete on Athletic Trainer System (ATS):
  1. ATS Registration

  3. Sickle Cell Tab
    1. For more information about how to obtain documentation, see Sickle Cell Trait Verification information under the efiles tab in ATS or view it here.
  4. Insurance Tab
    1. You must upload front and back pictures of your insurance card.
  5. Contact Tab
  6. Immunizations/Paperwork Tab
    1. Paperwork:
      1. Annual Medical History
      2. Concussion Policy
      3. Consent to Treat and Release of Medical and Treatment Information
        1. If under 18 years of age, parent/guardian signature required
      4. Mental Health Questionnaire
      5. Sports Medicine Policies
      6. Student-Athlete Insurance Coverage Acknowledgment
        1. Parent/Guardian/Policy Holder Signature required
  7. Forms
    1. View your completed forms/paperwork.
  8. eFiles
    1. Location for any additional print outs/instructions
    2. Upload Physical and Sickle Cell Trait Results
Upload to Athletic Trainer System (ATS):
  1. Physical- within 6 months of participation in any practice, competition, or out-of-season conditioning activity.
    1. Completed and signed by MD, DO, or NP.
      1. Note: You must use the Clark Sports Medicine form linked here
      2. Physical exams must be performed no earlier than the date listed below per sport:
        1. M/W Soccer, Field Hockey, Volleyball, M/W Cross Country: February, 18, 2026
        2. M/W Tennis, Rowing, M/W Swimming, M/W Basketball, M/W Lacrosse, Baseball, Softball: March 1, 2026
  2. Sickle Cell Trait Test Results
  3. If applicable:
    1. NCAA Banned Substance Exception- Asthma (form located in ATS or download it here)
    2. NCAA Banned Substance Exception- ADD/ADHD (form located in ATS or download it here)
    3. *NCAA Banned Substance Exceptions can be sent via Fax to: 508-793-7627
Concussion Baseline Testing
  1. Download the free Sway Medical Application to your mobile device.
  2. Your concussion baseline assessment will be completed with your team when you come to campus.

All NCAA forms must be completed in Teamworks. You may access Teamworks using Clark Single Source Sign on here: https://sso.armssoftware.com/Forms will be available in mid-July.

NCAA Form Checklist
  1. Student Athlete Information Form & NCAA ID Number
    1. All incoming students must get a NCAA ID number. You will need this to fill out your NCAA forms.
      1. If you do not already have one, sign up FOR A PROFILE ACCOUNT (Option #3) here: https://web3.ncaa.org/ecwr3/.
      2. Non-international Incoming Division III student-athlete DO NOT need to pay for an account.
      3. Incoming international students should contact Kirsten Clark for more information on requirements.
