Athletics Clearance Process For First-Years & Transfer Varsity Athletes
All incoming first year and transfer Student Athletes must complete two sets of forms - Medical forms and NCAA forms. Below are detailed directions showing how to complete your paperwork.
Medical Paperwork

Start by registering for Athletic Trainer System (ATS) software at
clarku2.atsusers.com.

Medical Clearance Checklist - Complete on Athletic Trainer System (ATS):

- You must upload front and back pictures of your insurance card.

- Upload Physical and Sickle Cell Trait Results
Upload to Athletic Trainer System (ATS):
- Physical- within 6 months of participation in any practice, competition, or out-of-season conditioning activity.
- Completed and signed by MD, DO, or NP.
- Note: You must use the Clark Sports Medicine form linked here
- Physical exams must be performed no earlier than the date listed below per sport:
- M/W Soccer, Field Hockey, Volleyball, M/W Cross Country: February, 18, 2026
- M/W Tennis, Rowing, M/W Swimming, M/W Basketball, M/W Lacrosse, Baseball, Softball: March 1, 2026
- Sickle Cell Trait Test Results
- If applicable:
- NCAA Banned Substance Exception- Asthma (form located in ATS or download it here)
- NCAA Banned Substance Exception- ADD/ADHD (form located in ATS or download it here)
- *NCAA Banned Substance Exceptions can be sent via Fax to: 508-793-7627
Concussion Baseline Testing
- Download the free Sway Medical Application to your mobile device.
- Your concussion baseline assessment will be completed with your team when you come to campus.

NCAA Form Checklist
- Student Athlete Information Form & NCAA ID Number
- All incoming students must get a NCAA ID number. You will need this to fill out your NCAA forms.
- If you do not already have one, sign up FOR A PROFILE ACCOUNT (Option #3) here: https://web3.ncaa.org/ecwr3/.
- Non-international Incoming Division III student-athlete DO NOT need to pay for an account.
